Stroke is very common and for most patients it means a major impact on quality of life and their independency in daily activities. Following stroke, the prognosis for recovery of movements of the leg are better than for movements of the upper extremity. Most patients keep limitations regarding movements of the paralyzed limb. Physical and occupational therapy treatments include more traditional concepts like Neuro Developmental Treatment (Bobath) Bobath or more recent treatment strategies like constraint induced movement therapy ("forced use"). Despite many existing treatment modalities, especially the rehabilitation of the upper extremity remains a major challenge.
Mirror Therapy (MT) or Mirror Visual Feedback is a relatively new approach in rehabilitation used in different neurological disorders, including stroke. During MT, the patient sits in front of a mirror that is oriented parallel to his midline blocking the view of the patient onto his affected arm. When looking into the mirror, the patient sees his unaffected arm positioned as the affected arm. By this, the patient gets the impression that actions upon his unaffected arm take place in his affected arm (see figure 1). Research suggests, that not every patient will profit by mirror therapy. Whether this might be the case depends on several aspects, such as cognitive functions, duration of symptoms or severity of paresis.
